The Provider Insurance page contains read-only information pertaining to the Provider's self-reported and/or verified detail of professional liability insurance, along with the insurance carrier's contact information.

Editable Fields available on the Insurance page are as follows:

Associated Addresses - Addresses shown on the Insurance page are those addresses specifically associated with the Client and which will be utilized for Provider Insurance update outreach attempts.  The User may edit those addresses / contact information shown with more current, information or edit the Address Type as Primary, Credentialing, Expirable-specific, etc. and/or Remove and Add (Additions to the Client Associated Addresses are performed on the Provider-Address page).
