Provider Search - Demographics

The Provider Demographics page contains information collected directly from the Provider's most current Initial or Recredentialing application or directly imported from the Client.  Due to data integrity restrictions, most fields of Provider data is read-only and unable for editing within G1 by the User.  

Editable Fields are as follows:

Provider Name (First, Middle, Last, Suffix) - By clicking on the "Edit" icon, the User may edit the Provider Name recognized by  the Client specifically.  NOTE: The Provider name default is the Provider name verified through NPPES (NPI Number). 

NPI Number - NPI Number may only be edited if the Provider NPI Number does not exist in the Provider's G1 record.  If an NPI number is currently shown on the Provider Demographics screen, it will be unable to be edited, as it has been verified with the NPPES and joined with other functionality within G1 hat utilizes the NPI Number.

CAQH Number - CAQH Number may only be edited if the Provider CAQH Number does not exist in the Provider's G1 record.  If a CAQH number is currently shown on the Provider Demographics screen, it will be unable to be edited, as it has been verified with the CAQH and shared with another Gemini Client.
